How to Submit a Warranty Claim

To begin a warranty claim, please email support@adaptivestar.com with the following information:

  • Clear photos of the complete chair (front, back, left side, and right side).

  • Close-up photos of the part requiring evaluation.

  • A brief description of the issue.

  • Your original Purchase Order (PO) number, dealer invoice, or proof of purchase.

  • Your name, organization (if applicable), phone number, and return shipping address.

Providing complete information helps us process your warranty claim as quickly as possible.

Repairs & Shipping

Many warranty issues can be resolved by shipping replacement components directly to you.

If additional evaluation is required, your chair may need to be returned to our service facility for inspection.

Before returning any chair or component, you must obtain a Customer Service Repair (CSR) Number by contacting support@adaptivestar.com.

If you purchased your chair through an authorized dealer, your warranty request must first be processed through that dealer.

Packages received without a valid CSR Number clearly marked on the outside of the box will be refused.

The dealer or customer is responsible for shipping the chair to Adaptive Star for warranty evaluation. International and Canadian shipments are also responsible for any applicable customs duties, brokerage fees, and taxes.

Once the warranty claim has been approved, Adaptive Star will repair or replace the covered item at our discretion and return it via standard UPS Ground shipping.

Typical repair and evaluation time is 10–15 business days after the chair is received at our facility. Turnaround times may vary depending on the repair required, and we will provide updates throughout the process.
